Public damage report

Report building damage

Tell us what you can observe from a safe location. You do not need engineering knowledge.

Report observations, not a diagnosis Your answers help Structiva coordinate next steps. They do not declare a building safe, unsafe, habitable, or uninhabitable.

Fields marked with * are required.

1

Immediate hazards

Start here so urgent danger is not buried later in the form.

Is anyone trapped or unable to exit safely? *
Does anyone have a serious injury related to the damage? *
Do you see an active fire? *
Is there a strong gas smell or suspected gas leak? *
Do you see sparking, electrical arcing, or dangerous fallen wires? *
2

Building location

Tell us where the affected property is. GPS is optional.

The list uses international ISO alpha-2 and alpha-3 country codes.

Enter it manually only when it is not listed. The location will be flagged for review.

The city is compared with geographic reference data stored in Structiva. If it is not listed, you may continue and the team will review the location.

What you can observe

Answer only from a safe location. “I don’t know” is acceptable.

Choose what best describes the main damage you can see. This identifies the report, but it is not a diagnosis or professional classification.

Do you see any part of the building collapsed or fallen? *
Does the building or any floor look clearly tilted, twisted, or displaced? *
Do you see new diagonal or X-shaped cracks? *
Do you see very open new cracks or visible separation between parts of the building? *
Are pieces of wall, facade, roof, or ceiling loose or falling? *
Do doors or windows that worked before now bind or fail to close? *
Did new cracks or settlement appear in the ground near the building? *
Do you see active ground movement affecting the building? *
Is the building close to a slope, embankment, or steep drop? *
Do you see soil or rocks that moved recently? *
Did the building already have unrepaired earthquake damage? *
Is a neighboring building significantly damaged? *
Does a neighboring building appear likely to fall toward this property? *
Are people currently inside the building? *
Did people leave or evacuate because of the observed damage? *
Has an authority already inspected or posted a notice on the building? *
Did new damage appear after a previous inspection or aftershock? *

Photos from a safe location

Do not move closer to damage just to take a photo.

Take photos only from a safe location. The platform will try to reduce their size before uploading them.

About the building

Approximate information is fine when you are not sure.

6

How we can reach you

Provide at least a phone number or email address.

We need at least a phone number or email address.

7

Request and review

Choose what you need, review the safety statements, and submit.

What do you need from this request? *

Before you submit

  • I answered from what I can safely observe.
  • I understand Structiva triage is not a professional structural classification.
  • If there is immediate danger, I will contact local emergency services or authorities instead of waiting for Structiva.

Photos are stored privately and used to manage this case. Your private tracking code is shown only after a successful submission.
